Snapshot: What is the f-SARA?

The functional Scale for the Assessment and Rating of Ataxia (f-SARA) is a scoring system to track how much a person’s balance and movement are affected. The f-SARA is an updated version of the original scale for ataxia, also known as SARA, especially designed for Spinocerebellar Ataxia. Traditionally, doctors measure the balance and movement in patients with ataxia via SARA score which includes eight different parameters such as how well you can follow the doctor’s finger with your hand or slide your heel down your leg. In contrast to this, f-SARA focuses more on how you move in your daily life. It is better at picking up small, gradual improvements over time, which is very helpful for researchers testing if a new medicine is working.

The f-SARA focuses on four key areas: walking, standing, sitting, and talking. To measure these, a doctor watches how steadily you walk a short distance and checks your balance while you stand in different positions, such as with your feet close together. They also observe whether you can sit upright without using your arms for support and listen to you speak to hear how clear and easy your words are to understand. Each task is then given a simple score based on your stability and whether you need any help to complete the movement safely.

In comparison to SARA, f-SARA has fewer tasks to be scored, such as the removal of finger-to-nose or finger-chase tests, which researchers found to be unproductive for measuring an affected person’s ability to perform daily activities. Additionally, f-SARA has a higher sensitivity in tracking small and meaningful changes over time, which provides a truer picture of the stage of the disease the individual is in. In short, f-SARA should be a more sensitive and practical version of the original SARA test, making it more productive and easier for doctors to observe whether the treatment is modulating a patient’s movement and speech.

In summary, because ataxia affects individuals differently and slowly progresses over time, it requires highly sensitive tests to track the changes. Measurable tests like f-SARA are designed to pick up and track the small changes in daily activities such as movement and speech. Highly sensitive tracking tests like f-SARA allow doctors to observe how individuals are doing and provide a true and clear picture of whether a new treatment is truly helpful or if the condition is progressing.
